1. Debulking surgery should be performed in cancer centers in cooperation with gynecologic oncologists, abdominal surgeons, and anesthesiologists.

Die Debulkingoperation ist sinnvollerweise in operativen Zentren interdisziplinär (Gynäkologischer Onkologe, Abdominalchirurg, Anästhesiologe) vorzunehmen.

2. PDT seems to be an adequate treatment for selected forms of early cancer and small lesions of the GI tract or for small residual areas after the tumour has been debulked by other techniques (e. g. limited surgical resection, thermal ablation).

Die PDT in kurativer Absicht empfiehlt sich ausschließlich in selektierten Fällen zur alleinigen Behandlung von Frühkarzinomen und kleinen Tumoren.

3. Five indications for irradiation are currently recognised: adjuvant irradiation of the inguinal regions following wide excision; adjuvant irradiation where the margins of resection are histologically positive; adjuvant irradiation after debulking tumorectomy; neoadjuvant irradiation for primarily inoperable disease; curative salvage irradiation following postoperative recurrence.

Heute bestehen 5 Bestrahlungsindikationen: adjuvante Bestrahlung der Leisten nach “wide excision”, adjuvante Bestrahlung bei histologisch positivem Resektionsrand, adjuvante Bestrahlung nach Debulking-Tumorektomie, neoadjuvante Bestrahlung bei primär inoperablem Befund, kurative Salvage-Bestrahlung nach postoperativem Rezidiv.
